Stated below are step-by-step instructions on how to upload the USPS Form 1583.

Why Must I Submit Form 1583?

Shop2Ship must have USPS Form 1583 on file in order to receive your mail. The Application for Delivery of Mail through Agent (Form 1583) shows that Shop2Ship is authorized to receive and forward mail for members with this form submitted.

Step 1

Download and Complete USPS Form 1583. Fill in all of the required fields.

Step 2

Have USPS Form 1583 Notarized. You can have your completed Form 1583 notarized by your local notary public, the US embassy or US consulate. Please have your Form notarized before submitting.

Step 3

You are required to submit two (2) forms of Photo Identification. Acceptable photo identifications include: passports, drivers license, state identification cards, student identifications, government employee IDs, corporate ID from place of business, current leases/ mortgages, voter registration cards, and home/vehicle insurance policies.

Step 4

Submit your USPS Form 1583 and 2 forms of Photo Identification. Ensure the USPS Form 1583 is notarized before submitting.
